Every failure in a lab or a pharmacy is a handover: bench to report, report to doctor, prescription to counter, counter to stock. These are the six that cost the most.
The instrument you already own
Laboratory platforms commonly price by how many analysers you may connect. Connecting an instrument you already paid for should not be a pricing tier, and here it is not.
The panic value at four o'clock
Flagged before the report is finalised, routed to the clinician who ordered it, and left outstanding until somebody acknowledges it. Not left on a screen for whoever happens to look.
The report that has to be corrected
A signed report is never silently edited. The correction publishes as an addendum against the original, so anybody who acted on the first version can see that it changed and why.
The batch that gets recalled
The pack handed over is the batch that was scanned, so the question "who received this lot" is a query rather than an afternoon in the store room with a notebook.
The test you send out
A referred-out test stays on the same order with its status visible at both ends, so it is not a blank line in the record until the partner lab happens to send something back.
